Job Title: Unit Manager
Summary/Objective:

Provides, plans, coordinates or manages nursing care and health education to nursing home residents on assigned unit throughout all hours of operation.

Essential Functions:

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Demonstrates the knowledge and clinical expertise required to effectively supervise all nursing activities in the facility or the units to which assigned.
  • Reviews and implements all nursing procedures and systems relative to the unit(s).
  • Assesses and evaluates the systems which facilitate the delivery of quality Resident care.
  • Assesses and evaluates the performance and competency of staff on unit(s).
  • Uses a systematic approach to provide individualized nursing care.
  • Performs nursing assessments regarding the health status of the Resident.
  • Contributes to the Resident’s assessment (MDS/CAA’s) and the development of a plan of care.
  • Develops a plan of care based on assessment, implementing nursing care.
  • Evaluates the Resident’s responses to nursing interventions.
  • Selects and institutes appropriate nursing intervention which might be required to stabilize a Resident’s condition and/or prevent complications.
  • Consults with physician or licensed independent practitioner.
  • Clarifies any order or treatment regimen believed to be inaccurate, non-effective or contraindicated by consulting with the appropriate licensed practitioner and notifying the ordering practitioner when making the decision not to administer medication or treatment.
  • Knows the rational for the effect of medications and treatments and correctly administer same.
  • Accurately reports and documents the Resident’s symptoms, responses and status.
  • Demonstrates measures to promote a safe environment for Residents and others.
  • Designs, promotes and organizes Resident education and counseling based on health needs.
  • Collaborates with the Resident, members of the healthcare team and, when appropriate, the Resident’s significant other(s) in the interest of the Resident’s healthcare.
  • Consults with, utilizes and initiates referrals to appropriate community agencies and healthcare resources to provide continuity of care.
  • Accepts only those nursing assignments that are commensurate with one’s own educational preparation, experience, knowledge and ability and obtains instruction and supervision as necessary when implementing nursing procedures or practices.
  • Uses independent judgment and discretion on behalf of the organization in the performance of these duties while adhering to facility policies and procedures.
  • Understands fire and emergency, safety and security procedures to include elopement.
  • Ensures that the charge nurses are adequately supervising the staff on their units.
  • Takes call as assigned.
  • Performs other duties as assigned.
  • Attend and participate in departmental meetings and in-services.

Supervisory Responsibility:

Supervises employees in the department and others for whom they are administratively or professionally responsible by following policies and applicable laws.
Certificates, Licenses, Registrations:

  • Current state license to practice as a nurse.
  • Current CPR certification and additional certification in nursing specialty desired.
  • Pass required background check.

Qualifications:

  • Credentials as a Registered Professional Nurse with a current New York State license required.
  • At least one year of experience as a Head Nurse or Charge Nurse preferable in geriatric nursing required.

Job Summary:
Responsible for nursing care rendered at facility during evening or night shifts; supervision of nursing personnel of each unit; and other corollary duties assigned by the Director of Nursing Services.

Job Duties:

  • Supervises Nursing care services and staff on all nursing units while on duty.
  • Monitors staff performance to ensure that plan of Care of all residents is being followed, and that Nursing care is properly documented.
  • Prepares the Twenty- Four (24) Status Report.
  • Assists with planning and daily coordination of staff schedules for assigned shifts.
  • Maintains Infectious Control Standards and provides education and support to staff and residents as needed.
  • Provides first aid and completion of Accident Incident Report for staff and visitors; explores Accident/incidents of residents as notified.
  • Coordinates laboratory, x-ray, oxygen services on the premises and records billing information for continuous oxygen use.
  • Assists with orientation and training programs for staff.
  • Conducts in service education programs, upon request.
  • Ensures that high standards of care are met, and that residents rights are protected and personal preferences are respected.
  • Carries out other work requests of the Director of Nursing Services.

This work involves the routine care of patients in accordance with detailed and well-defined rules and regulations. Performs routine nursing services in the care of sub-acute, chronic patients, acutely ill patients, does related work as required.

TYPICAL WORK ACTIVITIES:

  • Takes the temperature, notes the rate of pulse and respiration and observes and records other signs and symptoms as directed;

  • Carries out uncomplicated therapeutic treatments such as giving enemas, preparing and applying simple dressings and administering certain medications upon specific instructions of the physician.

MINIMUM QUALIFICATIONS:
At least one year minimum experience as a licensed RN, preferably in geriatric care.   • Responsibilities for MDS Coordinator:
  • Participate in the admission process of prospective residents in terms of their nursing. needs and appropriate ICD 10 codes for Medicare recipients.
  • Determine potential PDPM groupers and expense associated with a potential admission.
  • Complete and assure the accuracy of the MDS process for all residents.
  • Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
  • Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid reimbursement.
  • Facilitate problem-solving for complicated admissions.
  • Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission.
  • Qualifications for MDS Coordinator:
  • 4-years experience in MDS coordinator role or related experience.
  • Current and valid state nursing license.
  • Experience in a clinical and healthcare setting.
  • Deep medical terminology knowledge.
  • Exceptional mathematical skills and ability to apply them to tasks such as analysis of variance and sampling theory.
  • Excellent leadership skills.

Department: Nursing

DISTINGUISHING FEATURES OF THE CLASS: This work involves the routine care of patients in accordance with detailed and well-defined rules and regulations. Performs routine sub-professional nursing services in the care of sub-acute, chronic patients and assists a registered professional nurse in the case of acutely ill patients, does related work as required. In addition, a licensed practical nurse assists a registered professional nurse in the carrying out of orders prescribed by a physician.

Position Summary:

  • The Cook prepares and serves food including texture modified and therapeutic diets according to the approved menu.
  • Prepares food in accordance with current applicable federal, state, and local standards, guidelines and regulations, in line with our established policies and procedures
  • Clean and sanitize kitchen facilities, utensils and equipment.
  • The Cook assists in assuring proper receiving, storage, preparation, serving, sanitation, and cleaning procedures are followed.
  • The Cook must be detail-oriented, well-groomed, act professionally and work well with others.
  • The Cook must assist dietary aides as necessary, and direct other employees with approval and in the absence of the Food Services Director and the Food Service Supervisor.

Job Summary:

The primary purpose of your job position is to perform the day-to-day activities of the Housekeeping Department in accordance with current federal, state, and local standards, guidelines and regulations governing our facility, and as may be directed by the Administrator, and/or the Housekeeping Supervisor, to assure that our facility is maintained in a clean, safe, and comfortable manner.

Job Responsibilities:

  • The Housekeeper assumes daily responsibility in the following areas:
  • Perform day-to-day housekeeping functions as assigned.
  • Perform specific tasks in accordance with daily work assignments.
  • Ensure that assigned work areas are maintained in a clean, safe, comfortable, and attractive manner.
  • Ensure that that equipment, tools, supplies, etc., are properly stored at all times.
